A client maybe referred to by their physician, hospitals, nursing homes, insurance companies, care givers, family, friends as well as self-referral. Accepted referrals are admitted to our services 7 days a week. All referrals should be faxed to: Fax@Amarisedinc.com or our 24 hour referral line at (413)557-4924.

STEP 2: Make sure all information is filled out
STEP 3: Make sure the form has the Doctor’s signature
STEP 4: Send Diagnosis
STEP 5: Send Notes & Date of Last Visit
STEP 6: Make sure to check patient’s Home Bound Status
STEP 7: Check Both Boxes on the reason for referral section
